0-25 Integrated Special Educational Needs and Disabilities Team

Who are we?

Our team comprises of 13 members:

  • A lead SEN officer who is responsible for overseeing the team and for decision-making
  • Two complex case co-ordinators.
  • Three case coordinators who are responsible for putting together Education Health and Care Plans and ensuring Assessments of Need run to the timescales as laid down in the SEND Code of Practice.
  • Two case coordinators who are responsible for converting the Statements of Special Educational Needs from the previous legislation to Education Health and Care Plans, focussing on children in Years six and 11.
  • Three team coordinators who work closely with the case coordinators responsible for new assessments and who liaise with internal and external agencies to obtain the reports requested as part of the Assessment of Need.
  • Two team coordinators who work closely with the case coordinators responsible for conversion and who liaise with internal and external agencies to obtain the needed to enable the Statements to be converted to an Education Health and Care Plan.

The Team as a whole is also supported by a SEND Business manager and Modern Apprentices.
