Templates and referral forms

EHC Needs Assessment Request templates

These documents provide a template for an education setting, parent or young person to request a EHC Needs Assessment for a learner. If a school and parent, carer or young person are submitting a request jointly the following document should be completed using the guidance document for support -

 



If a parent wishes to request an EHC Needs Assessment the following document should be completed -

These documents are used to gather information from those involved in supporting a learner if it is agreed to carry out an EHC Needs Assessment.

The person convening the EHC Review Meeting (usually the educational setting) will be responsible for completing the review documentation below using the guidance provided.

For Learners below Year 9:

For Learners in Year 9 and above:


SEND Transport Review Form 

Where a learner with an EHCP is in receipt of Local Authority Funded Transport the Transport Review Form must also be completed as part of the EHC review. A copy of this form can be found below:  


Requesting an Increase in Banding 

If an educational setting feels it is necessary to request an increase in the banding allocated to support a learning with an EHCP, settings should give due consideration to the following guidance:  




Mental Capacity Act Information Leaflet 

This information leaflet outlines how the Mental Capacity Act (2005) impacts on the decision-making responsibilities in relation to the EHC processes when a learner turns 16 years old.  


 

This area has been developed to provide colleagues with possible templates which they may wish to use in their settings to support SEND Practices. Educational settings are welcome to use these templates as they are or edit them to better suit the processes and terminology used within their settings.

Annual Review Invite Letter Templates

These letters can be used to inform others of the planned date of an EHC Review and request reports and a second letter to accompany EHC Review Reports which must be circulated with the formal invite at least two weeks before the meeting.

 

Cause for Concern Forms

These forms can be used to ensure an accurate record is kept of initial concerns relating to a learners possible Special Educational Needs.

One Page Profiles

One Page Profiles (sometimes referred to as pupil passports) can be a really useful way of summarising a learners’ needs and associated provision.

Recording SEND Support, Individual Education Plans, Support Plans

These documents can be used to record the Assess, Plan, Do, Review Cycles which take place for a learner with SEND. These should be shared with parents, carers or young person on a termly basis.

Transition

These documents may be useful to record support through a period of transition.

Vulnerable Placement Notification 

If an educational setting is becoming concerned about the stability of a placement of a learner with an EHCP within their setting. They are able to notify and request support from the SEND Assessment and Review Team by completing the form below using the guidance provided:  



Funding Requests

Educational Settings may at times feel they need to seek additional financial support from the Local Authority. The following forms can be used to support such requests:

Covid 19 Short Term Funding

Where schools believe some short term additional funding may support learners with an EHCP to reengage in education following the Covid Pandemic they can request some additional short term funding using the following form:
