This area has been developed to provide colleagues with possible templates which they may wish to use in their settings to support SEND Practices. Educational settings are welcome to use these templates as they are or edit them to better suit the processes and terminology used within their settings.
These letters can be used to inform others of the planned date of an EHC Review and request reports and a second letter to accompany EHC Review Reports which must be circulated with the formal invite at least two weeks before the meeting.
If an educational setting is becoming concerned about the stability of a placement of a learner with an EHCP within their setting. They are able to notify and request support from the SEND Assessment and Review Team by completing the form below using the guidance provided:

Where schools believe some short term additional funding may support learners with an EHCP to reengage in education following the Covid Pandemic they can request some additional short term funding using the following form:
