This area has been developed to provide colleagues with possible templates which they may wish to use in their settings to support SEND Practices. Educational settings are welcome to use these templates as they are or edit them to better suit the processes and terminology used within their settings.
These letters can be used to inform others of the planned date of an EHC Review and request reports and a second letter to accompany EHC Review Reports which must be circulated with the formal invite at least two weeks before the meeting.
These forms can be used to ensure an accurate record is kept of initial concerns relating to a learners possible Special Educational Needs.
One Page Profiles (sometimes referred to as pupil passports) can be a really useful way of summarising a learners’ needs and associated provision.
These documents can be used to record the Assess, Plan, Do, Review Cycles which take place for a learner with SEND. These should be shared with parents, carers or young person on a termly basis.
These documents may be useful to record support through a period of transition.
