Speech and Language Therapy – New Referral Forms

This service has updated their referral forms to be more user friendly and accessible. The new layout allows the referral agent to provide more relevant information about the child/young person’s speech, language and communication needs as well as strategies and activities which have already been tried.

The referral forms can be found in the usual place on the Speech and Language Therapy website Paediatric Speech and Language Therapy (humber.nhs.uk) A link to this website can also be found from the Hull Local Offer website.

There will be a video clip on the Speech and Language Therapy website coming soon demonstrating how to complete each section.
