SEND team and SEND updates

The SEND assessment and review team are responsible for making decisions around children and young people whose special educational needs require an Educational Health and Care (EHC) Plan to be in place. If an EHC Plan is determined to be issued then the team are responsive for the continuing administration and any amendments to the content of the plan during its life time which can potentially be up to 24 years. The team liaise closely with other services such as health, social care and education settings in order to ensure that the special education provision indicated within any EHC Plan is being delivered. 

The child/young person needs a learning difficulty or disability that requires provision that needs securing by an ECH plan. The provision is additional to or different from that which is ordinarily available as universal provision in schools.

Ages 0-25 years

The team receive requests for EHC Plans from education settings, parents and other professionals such as social care. 

The service can be accessed Monday- Thursday 9-5 and Friday 9 - 4.00.
